· Create a chart including at least 12 local, state, and federal resources that are available to address this issue.

· Include at least 3 resources at each level.

· Be specific (i.e., don’t just say local health department; instead, say Cumberland County Public Health Department).

· Include a link and/or phone number for each resource that you find.

You can keep it very simple (like the example below), or you are welcome to be creative and create a product that you could use as a handout to families needing resources.
