This journal assignment is designed to help you critically reflect on your preceptorship, apply

theoretical knowledge to practical experiences, and develop your professional judgment and

skills in nursing practice. Each journal entry should combine description, analysis, and reflection

to comprehensively view your experiences.

Instructions

Reflect on your clinical experiences in the medical-surgical nursing unit.

• Reflect on your experiences during your preceptorship rotation. For each entry, describe

the situation, analyze your actions and their outcomes, and reflect on what you learned

and how you could improve.

• Write a journal entry for each prompt below. Each response should be about 200-300

words.

• Ensure your journal is professionally formatted, proofread, and submitted by the due

date.

• Submit your journal as a single document, each entry titled and separated.

Prompt 1: Therapeutic Communication ( mother of 21 yr old male dealt with loss of child due to drug over dose ED team coded patient for 30+ min patient was down for unknown time. mother did not want to loss child however quality of life was in question by the mother on how he would be if we were able to get him back. ) 

Objective: Utilize therapeutic communication techniques to effectively support clients and their

families, addressing their concerns with empathy and professionalism.

• Describe a challenging communication event with a patient or family member.

• Analyze the verbal and non-verbal strategies you used.

• Reflect on the patient’s response and discuss how effectively your approach supported

the patient’s emotional or psychological needs

Prompt 2: Clinical Knowledge and Application (  I dealt with several patient with chest pain DKA, Drug overdose ) 

Objective: Apply advanced knowledge of pathophysiology to prioritize and execute interventions

for multiple clients, demonstrating a clear understanding of the physiological basis for each

action.

• Reflect on a critical care situation involving multiple clients.

• Detail the pathophysiological considerations that guided your prioritization of care.

• Evaluate the effectiveness of the interventions you selected and identify any gaps in your

knowledge that became apparent.

Prompt 3: Client Care Prioritization ( I cared for patient with Chest pain, abd pain, bump/lumps and cold)

Objective: Prioritize the delivery of client care by assessing the urgency and importance of

various tasks and interventions, ensuring efficient and effective patient management.

• Analyze a day with high patient turnover or multiple emergencies.

• Describe how you managed your responsibilities and prioritized tasks.

• Reflect on any ethical dilemmas you faced and how you resolved them.

Prompt 4: Evaluation and Documentation ( really didn’t get to chart much but they did show us how their system ran and where to find all the results and place orders in, I did eval pain before and after pain medication along with bp medication to either increase or decrease BP gave blood and fluids) 

Objective: Critically evaluate and accurately document the client’s treatment responses, focusing

on managing side effects and adverse reactions to medications and therapies.

• Discuss a specific instance where you had to monitor a patient closely for adverse

reactions to medications or treatments.

• Describe the documentation process and how it was used to adjust the care plan.

• Reflect on the importance of detailed and accurate documentation in patient care.

Prompt 5: Interprofessional Collaboration ( worked PA , NP, lab, PT/OT, wound care,  case management for referral to home health)

Objective: Collaborate with interprofessional team members to provide comprehensive client

care, sharing insights and integrating feedback to enhance patient outcomes.

• Provide an example of how you worked with another healthcare professional to manage a

patient’s care.

• Analyze the dynamics of this collaboration and reflect on how interprofessional

teamwork impacted the patient outcome.

Prompt 6: Delegation and Supervision ( have AUA/ CNA and ED tech along with LPN that assist with lab draws,  EKG collecting urine specimen transporting patient to and from imaging )

Objective: Demonstrate understanding and application of the rights of delegation, ensuring tasks

are appropriately assigned and supervised following client needs, situational demands, and team

capabilities

• Reflect on an experience where you delegated tasks to other team members.

• Discuss the criteria you used to assign tasks and evaluate the effectiveness of the

delegation, including any feedback you provided or received.

Prompt 7: Cultural Competence  ( we had serval patient with different cultural backgrounds children taking care of elder parents and families with different languages and had to use medical translator. )

Objective: Display cultural sensitivity and competence in all interactions with clients and their

families, tailoring communication and care practices to respect diverse backgrounds and values.

• Describe an encounter with a patient whose cultural background differed significantly

from yours.

• Discuss how you adapted your communication and care approaches.

• Reflect on this experience and how it has affected your approach to culturally competent

care.

Prompt 8: Communication of Care Tasks ( Communication with dr and floor staff and surgery team) 

Objective: Effectively communicate tasks and critical client concerns to team members and

supervisors promptly, ensuring clarity and timeliness in the delivery of care.

• Analyze a situation where effective communication was crucial to patient safety.

• Describe the specific communication techniques you used, the immediate outcomes, and

how you could improve future communications.

Prompt 9: Education ( Education to family and pt about medication with adherence to medication such as eliquis, bp medication like metoprolol, and insulin for diabetes)

Objective: Educate patients and their families about health conditions, care routines, and

preventative measures, employing clear, understandable language and materials suitable for their

levels of comprehension

• Detail a situation where you educated a patient or their family about a health condition or

care procedures.

• Evaluate the clarity and effectiveness of your educational delivery and reflect on how you

could enhance patient comprehension and engagement in the future.

Prompt 10: Confidentiality and Privacy ( receiving and giving report in a not public area to keep patient info confidential. and adhere to HIPPA)

Objective: Maintain strict confidentiality and privacy of client information, adhering to ethical

guidelines and legal requirements to protect patient rights and sensitive data.

• Reflect on a situation where maintaining patient confidentiality and privacy was

challenging.

• Discuss the steps you took to secure patient information and evaluate how well you think

you managed these responsibilities.

Submission

• Submit your completed reflection essay no later than 48 hours after your last

Preceptorship day.

• Ensure that your responses are well-organized, clearly written, and free of grammatical

errors.

• Include any references to literature or course materials in APA format.
