– Identify at least TWO assessments that can be used to determine whether a patient is safe in their current living environment?

      There are multiple ways to assess patients’ safety and living environment. In this regard, environmental safety evaluation and functional assessment are important. Environmental safety methods identify hazards like poor lighting, uneven flooring, and inaccessible bathroom facilities that could endanger the patients. However, the functional evaluation deals with the patient’s ability to perform necessary tasks such as bathing, cooking, dressing, and managing medications (Kon & Wood, 2022).

1. Discuss results that indicate the patient may require additional assistance or a transition in the care environment.

             There are various indicators that reflect that the patient requires additional care. For instance, a decline in activities of daily living scores indicates reduced independence of patients. Similarly, signs of neglect like poor hygiene and malnutrition also indicate patients’ additional care. In addition, cognitive impairments such as dementia or confusion also require additional care.

1. Describe the Nurse Practitioner’s role in working with the patient and family to assist with care transitions.

The role of nurse practitioner in working with patients and families cannot be ignored. Nurse practitioners assess the patient’s needs ensuring that they align with the transition plan. Similarly, they also educate the patient and family about home care services and coordinate with interdisciplinary teams to improve the patient’s health and develop a seamless transition plan.

 

1. Identify at least TWO appropriate referrals for a patient who requires a transition in the care environment. How does the Nurse Practitioner ensure coordination of care during transitions?

As far as the referrals for a patient are concerned, home health services are quite useful in this context. Community social services also play a vital role in assisting financial and housing resources. Social workers also play a significant role in connecting the patient with the community. No doubt, nurse practitioners ensure a smooth transition by providing comprehensive handoffs, sharing medical records, and maintaining communication with new care providers (Razon et al., 2020).

1. Identify at least TWO public or private resources that are available for patients with limited resources in your intended practice area.

There are multiple public or private resources that are available for patients in San Diego. Firstly, Aging and Independent Services (AIS) offers care management, home modifications, and meals for seniors with limited resources. In addition to this, 211 San Diego is also an important resource that connects individual programs for food, housing, and healthcare support (Lewis et al., 2024).
