Create an outline or a graphic organizer of the following community assessment plan to prepare for the following week’s video presentation. 

Details

For this assignment, identify an area of focus in community health within your community. 

Using Gordon’s functional health patterns framework in your e-text, assess the health risks in your community.

In your community assessment outline, include the following:

· Identify resources in your community that would enable you to complete a community assessment and submit a summary of your findings.

· Refer to Table 6.2: Examples of community strengths and concerns and assess the strengths and concerns of your community.

· Identify potential barriers to implementing community health plans in your community and brainstorm ways of addressing these barriers.

Findings from your assessment may include the following:

· Using prenatal and infancy home visits to prevent child abuse and neglect

· Preventing

· Falls in the elderly

· Risky sexual behavior among youth and young adults

· Improving vaccination rates

· Reducing

· Population salt intake

· Tobacco use among adults

· Drug experimentation among young adults

· Motor vehicle accidents

· Physical abuse among partners

You must include 2–3 sources cited and referenced in APA Style. Sources may include community resources such as flyers, brochures, interviews, news stories, and local research data from credible sources.
